An employee was attempting to pull out a blank from a machine when his right middle finger became caught between the feed rollers. His right middle fingertip was amputated.

An employee drove a stand-up straddle truck toward a table to get ear plugs. The employee tried to stop the powered industrial truck three times, but it did not stop and ran into the table. The employee suffered six broken ribs, a fractured clavicle, slightly bruised liver, and a 10 percent collapsed lung.
