On 12/13/2019 at 9:55 a.m., an employee was packing items for customers at her workstation when she tripped over a box on the ground and fell onto the concrete floor. She suffered a right ankle fracture and distal fractures to the tibia and fibula, requiring hospitalization and surgery.
An employee operating a powered industrial vehicle stopped and leaned out of the operating compartment to scan a product with a scan gun. Her left hand made contact with the truck's operating controls which caused it to move. She sustained a partial amputation to her left finger.
