An employee had changed out belts/sheaves on fan motors for a dry kiln machine. While the employee was manually turning the belt to test the tension, his left index finger got caught between the belt and a sheave and was amputated.

A material handler from Remedy Intelligent Staffing was using a forklift to load 95-pound rectangular tubing (4 feet long, 4 inches high, and 6 inches wide) onto a cantilever rack. He got off of the forklift to push the metal tubes onto the rack when one of the tubes fell between the cantilever arms and his right middle finger, causing the amputation of the fingertip.

An employee was positioning an angle iron to set a stop for punching holes. The punch machine was engaged, crushing his left middle finger and necessitating a medical amputation.

An employee was using a 7.5-ton overhead crane to move a suspended load while holding onto the load with his right hand. The load weight plate (with his hand on it) then struck the back of a forklift, and his right index and middle fingers were lacerated. He was hospitalized.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

On May 9, 2025, an employee was using an air hose to clear debris from the courtyard area when she slipped and fell. The employee was hospitalized with closed fractures to her left fibula, tibia, and medial malleolus.

On April 24, 2025, an employee was opening a cylinder door on a hydraulic bridge to remove lumber when their left middle finger was caught between the door and the hydraulic bridge, resulting in an amputation without bone loss.

Two employees were using a plywood chop saw. The injured employee was holding one end of the saw with their right hand when the operator on the other end pressed the start button. The saw blade contacted the injured employee's right hand, resulting in multiple finger amputations when it amputated the hand above the thumb.

On January 17, 2025, at 12:15 p.m., an employee was moving 11-gauge steel sheets from a holding cart using a pendant-controlled, 1-ton jib hoist equipped with a plate clamp lifting attachment. With the plate clamp empty and open, the employee began raising the hoist. The clamp became caught on the bottom of the cart, tipping the cart in the direction of the employee. Several sheets of steel slid off the cart and struck the employee, resulting in several broken ribs, a broken left leg and ankle, a fractured kneecap, and a torn left ACL. The employee was hospitalized.

An employee was operating an air hoist to rotate a blending mill counter-clockwise. After he connected the hook to the mill, the hoist was raised and his hand was pinched between the hook block and the ring on the mill. The employee sustained a partial amputation of his right thumb.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
