An employee was stepping off of a 24" tall platform when he fell and landed on a 1" steel pipe, suffering fractured ribs and a perforated lung.

An employee was utilizing a 20-ton overhead crane to place a steel insert into the hull of a ship. The employee attached the crane to the pad eye and began lifting the steel plate. The pad eye failed, releasing from the steel plate and causing it to strike the employee. They sustained a laceration to the right side of the face/head, requiring hospitalization.

An employee was operating a steel roller machine. The rollers caught the employee's glove. The machine stopped. When it was reactivated, it pulled in and partially amputated the employee's right index finger.

An employee was walking down a ship's ladder carrying an 8-foot-long scaffold pole. Her foot slipped on the wet steps of the ship's ladder and she fell 6-7 feet, striking several metal surfaces. The employee suffered a scalp laceration, four broken ribs, and fractures to three vertebrae in her lower back.

An employee was unclogging a pneumatic paint gun when bronze paint was discharged into a finger on the employee's right hand. The employee was hospitalized.

An employee was repairing an abrasive blasting pressure pot. The employee turned off the blasting machine before leaving the area to get a different tool. While the employee was away, a coworker turned the machine back on. When the employee returned to continue work, the coworker triggered the system, striking the employee with pressurized blast media. The employee suffered a serious abrasion and punctures to the face, neck, torso, arms, and leg, requiring hospitalization. The machine was not locked/tagged out at the time of the incident.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee was preparing to bend a flat bar in a brake press. When the machine was jogged, the stock rotated up and crushed his left middle fingertip against the outer frame of the die. The fingertip was amputated.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
