A truck driver was in the back of a trailer, clearing it out after a delivery. The driver began to climb out of the trailer using a ladder leaning against it; the ladder kicked out and the employee fell to the pavement, suffering a broken hip and bruised ribs.

Two employees were changing the grinding hammers on a tub grinder that is used to grind mulch. As one employee was using a sledgehammer to pound one of the grinding hammers, a piece of the sledgehammer chipped off and struck the second employee in the arm, cutting an artery and requiring surgery.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
