An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was working on unscrewing a knee wall panel (corrugated metal) next to an unguarded stairwell in order to expose the structure behind it. He lost his balance and fell 7 feet to the concrete landing of the stairwell. He was hospitalized with lacerations on the front and back of his head, fractures to both forearms, and a knee injury.

CTA Architects P.C.

An employee was conducting an exterior inspection of a building. A fire escape was used to access the facade of the building. When the inspection was completed, the employee turned around at the edge of the fire escape platform to descend the ladder to the ground floor. A construction worker was descending the fire escape from the floor above. There were vibrations, and the employee's right foot gave out, causing them to fall approximately 10 feet to the ground. The employee sustained fractures to their nose and right cheekbone, and three fractures to the pelvis. The employee was hospitalized and required surgery.

The employee sustained a head injury including loss of consciousness, bleeding, bruising, and facial fractures.

The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
