Federal OSHA recorded a severe workplace injury at Alfred Benesch and Company, 4349 S 90th St, OMAHA, NEBRASKA 68127 on — Injuries to the brain, spinal cord and severe wounds, internal injuries, affecting the Multiple regions of the head.

An employee was operating a pedestal-mounted core drill to obtain core samples. The employee had reached the bottom of the concrete pavement while operating the drill and the drill bit stopped spinning. After reengaging the drilling process, the injured employee raised the drill bit halfway out of the concrete pavement and fell off the pedestal plate, hitting his head on the concrete pavement. The employee sustained a head injury including loss of consciousness, bleeding, bruising, and facial fractures.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Kisatchie Midnight Express

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Lockheed Martin Mission Systems and Training

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

Kenrick Design/Construction Services, Inc.

An employee was working on unscrewing a knee wall panel (corrugated metal) next to an unguarded stairwell in order to expose the structure behind it. He lost his balance and fell 7 feet to the concrete landing of the stairwell. He was hospitalized with lacerations on the front and back of his head, fractures to both forearms, and a knee injury.

CTA Architects P.C.

An employee was conducting an exterior inspection of a building. A fire escape was used to access the facade of the building. When the inspection was completed, the employee turned around at the edge of the fire escape platform to descend the ladder to the ground floor. A construction worker was descending the fire escape from the floor above. There were vibrations, and the employee's right foot gave out, causing them to fall approximately 10 feet to the ground. The employee sustained fractures to their nose and right cheekbone, and three fractures to the pelvis. The employee was hospitalized and required surgery.

On March 5, 2025, an employee was lifting a 49-pound roller die onto an angle roller machine. The die dropped while they were setting it down. It landed on their right little finger, amputating some of the tissue on the finger. The employee sustained a medical amputation of the finger including bone.

An employee was washing brick with a cleaning chemical containing hydrofluoric acid. The chemical penetrated his glove and burned his right hand.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
