An employee was standing on the second landing of a recently poured concrete staircase, cleaning mortar droppings from handrails, when the employee lost balance and slipped/fell down six concrete steps.
An employee was working on the second level of a scaffold approximately eight feet high. The employee was reaching up to adjust the toe board for the level above when he slipped and fell through the mid-rail to the ground below. The employee suffered a left shoulder and elbow injury and a laceration above the left eye requiring stitches.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
