An employee was preparing the serving line area for the evening meal. He was cleaning a stainless steel table when his left middle finger got stuck in a gap where a raised ledge is affixed to the table. As he pulled his finger from the space, his fingertip was amputated.

An employee was unloading goods from a pallet and putting them onto metal shelves when their wedding ring got caught on a shelf and their finger was amputated.

An employee was in the back of a dump truck emptying a bag from a mower. When he placed his hand on the side of the truck and as he came down from the truck, his ring became caught. The employee sustained an amputation to his ring finger.

On May 18, 2023, a maintenance employee was descending a fixed ladder when his ring was caught on a ladder rung. The employee sustained a fracture to the left ring finger that required surgery.

An employee was unloading a pipe from a fitted rack on his truck. As the employee grabbed a pipe strap, his glove became caught. The employee then went to step off the back of the truck and the top inch of his left hand ring finger was partially amputated.

An employee was repairing and testing a side-by-side utility terrain vehicle. The vehicle rolled over, and the employee suffered two broken ribs.

An employee was lying on the ground behind the drop axel of a semi-truck while checking the brake chambers when the drop axel lowered due to the engine shutting off. The axel struck the injured employee and pinned them against the floor, resulting in broken ribs and a swollen eye.

An employee was ejected from a golf cart that flipped over while going up an incline. The employee was hospitalized for pelvic fractures.

An employee was on a ladder removing a pin that was stuck on a garage door when the door released and knocked them off of the ladder, causing them to fall 10 feet. They sustained fractures to two ribs, a scapula, and their skull.

A fuel truck operator was crawling under a fuel truck to put down an absorbent pad, due to a fuel leak. The employee was caught in a rotating shaft under the truck and sustained a broken ribs, a fractured vertebra, and a right shoulder injury.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
