An employee was conducting jail cell window bar taps. They were closing one cell and opening the adjacent cell at the same time and sustained an electrical shock from an energized metal door.

An employee was serving food in the kitchen serving line. An apple fell while being served. The employee slipped on the apple and fell to the ground. The employee sustained fractures to their head and ankle requiring hospitalization.

An employee was participating in firearms training when she sustained a gunshot to her left hand from a pistol, fracturing her index finger.

An employee was using a table saw and cut the three middle fingers on his left hand. He suffered a partial amputation to the ring finger.

On September 15, 2023, an employee was using a table saw to cut a piece of wood when a kickback occurred, causing the employee to lose balance. They reached to catch themselves and the blade of the table saw amputated their thumb and index finger.

An employee was unloading goods from a pallet and putting them onto metal shelves when their wedding ring got caught on a shelf and their finger was amputated.

An employee was in the back of a dump truck emptying a bag from a mower. When he placed his hand on the side of the truck and as he came down from the truck, his ring became caught. The employee sustained an amputation to his ring finger.

On May 18, 2023, a maintenance employee was descending a fixed ladder when his ring was caught on a ladder rung. The employee sustained a fracture to the left ring finger that required surgery.

An employee was unloading a pipe from a fitted rack on his truck. As the employee grabbed a pipe strap, his glove became caught. The employee then went to step off the back of the truck and the top inch of his left hand ring finger was partially amputated.

On April 26, 2023, at 9:50 AM, an employee was installing a ladder rack on a company vehicle when the lowering arm of the ladder rack shifted, causing a bolt to slip and catch the wedding ring on his left ring finger. The employee sustained a laceration/avulsion to the left ring finger between the first and second knuckles that required hospitalization.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was responding to a physical altercation between two inmates when they slipped and fell on the ground, landing on their left arm. The employee was hospitalized with a compound fracture to their left forearm.

An employee was working on an out-of-service forklift when his hand was caught in the door frame as the stuck cab door was being opened. The employee sustained a middle fingertip amputation.

A load bearing pin was stuck. The injured employee went to remove the pin manually as a second team member applied pressure from the opposite side. The pin unexpectedly released and struck the injured employee s right thumb, resulting in partial amputation of the distal phalanx including an open distal phalanx fracture and nail bed laceration.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

An employee was preparing a bundle of green onions for chopping. While holding the bundle in his left hand, he made his first cut using a 9-inch kitchen knife held in his right hand. The knife contacted the tip of his left thumb, resulting in an amputation of approximately 0.5 inches of the thumb that required hospitalization.
