Final narrative
An employee was attempting to blow out the core box mold to make sure it was clear from residual sand when the machine cycled unexpectedly, pinching his left index finger between the pin and the cup. His finger was amputated at the first knuckle (mid-nail). The machine was in automatic mode at the time of the incident.
An employee was using a slag spoon to skim off impurities on molten gray iron. The employee dipped the spoon into the zircon wash and gray iron popped onto the employee. The employee sustained first and second degree burns to the left side of the palm, the forearm, and the abdomen.
