After moving a box from one pallet to another, an employee turned to walk away; her foot caught in wrapping on a pallet and she fell, suffering a broken hip.
An employee was operating an electric pallet jack in a warehouse. As he turned out of one aisle into another, he had to reverse to get a better angle. While reversing, the pallet jack struck a metal guardrail and trapped the employee's right ankle against it. The employee's foot was broken.
On May 29, 2023, an employee was making a 180-degree turn to align his forklift with the motion sensor on the door of the perishable warehouse. Their forklift struck another stationary forklift and the employee's left ankle was punctured by the raised forks of the stationary forklift.
