First Solar Inc.

A post pounding machine was being used to pound a post on site. The operator lifted the hammer top high and the hammer came out of the mast. The mast assembly missed an in-plate that would have prevented the hammer from separating from the mast when lifted above a certain height. The hammer struck the employee while he was setting up another post for placement. The hammer fell onto his hard hat and grazed his lip/chin area. The impact caused significant lacerations to the head and face.

On April 18, 2018, an employee was helping to clean out heat exchangers when a metal rod attached to a cordless drill used to clean salt deposits slipped and fractured/lacerated the employee's right thumb.

An employee was lifting a motor and reaching to place it on a machine when they sustained a back injury. The employee was hospitalized.

On 12/22/2023, an employee was carrying bins filled with pasta to a refrigerator. As he lifted a 20-pound bin, he felt pain in his lower back. The employee sustained a herniated disc.

An employee was lifting a patient when they pulled a back muscle. She suffered back pain and spasms radiating into her leg with weakness and numbness. The employee's L3-L4 disc was herniated and she was hospitalized.

An employee was lifting boxes when they suffered a strained back, resulting in hospitalization.

An employee was lifting a bundle of insulation with a colleague when he felt severe pain in the right side of his lower back/hip region. The employee sustained a severe muscle injury.

CONTINENTAL AUTOMOTIVE SYSTEMS

An employee positioned a pallet jack under a mobile TV stand to lift the stand and then clean the wheels. As the employee raised the pallet jack, the TV stand fell forward toward the handle of the pallet jack. The employee went to catch the TV stand and their right thumb became caught between the TV mount frame and the pallet jack, resulting in amputation.

Sinbon Ohio, LLC

An employee was working on the crimp machine when their left middle finger became caught in the machine. The employee sustained the amputation of the fingertip through part of the nail and was hospitalized. The machine was guarded at the time.

Semiconductor Components Industries, LLC

An employee was greasing the belt of an air handler and manually moving the belt when their left ring finger was caught between the belt and the pulley. The employee sustained a severe laceration to the fingertip that resulted in an amputation.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
