Final narrative
Two employees were diagnosing a bundling machine and were in the process of removing blocks. When the employees actuated the bundling machine, the machine lifted but the machine frame did not. One employee had his fingers between the frame and machine at the time, resulting in the amputation of the employee's right middle and ring fingers.
An employee was removing a battery from a side loader. The battery cover came down and crushed the employee's left middle finger, causing an amputation from the back of the nailbed to the fingertip.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
