Final narrative
Two employees were performing a changeover of steel rolls/tooling. While one employee was pushing a metal roll stand back onto a shaft, the injured employee's left middle finger was caught between the metal spacer and the key-way of the metal stand. The fingertip was amputated.

An employee was operating a tube cut-off machine when there was a mis-cut resulting in a longer piece of tubing exiting the machine. The operator saw the mis-cut and reached to remove the tubing. The next tube to be cut struck the end of the tube the operator was trying to remove, causing the employee's fingers to be pinned between the end of the tube and the exit chute. The employee sustained amputation of his left index finger.
