An employee's right thumb was lacerated by the stream of pressurized water while power washing a deck.

An employee was using a forklift to stack hay bales when some hay bales fell onto the cage. The employee was ejected from the forklift and landed on the ground, resulting in head and neck trauma that required hospitalization.

An employee was using a powered pallet jack to move a fixture. His leg became caught between the pallet jack and the fixture and he fell on the same level, resulting in a fractured left fibula.

An employee was operating a forklift in a warehouse when it began to slide. The forklift was unable to gain traction on the floor that was wet from rain and it slid out of the building and onto the employee's foot. The employee sustained a crush injury to the right foot.

An employee was driving an ATV on a private roadway to respond to a well control event. The employee lost control of the ATV and used their left leg to try and prevent the ATV from flipping over. The employee's left leg was fractured.

An employee was operating a powered industrial truck when it went out of control and pinned his ankle and foot, breaking them.

An employee was driving a 4-seat ATV from one field to the next field on the site. They went around a corner and glare from the sun blocked the employee's sight. The ATV struck a parked car. The employee sustained a broken hip and a laceration to the head.

An employee was repairing the hydronic heating system for the building. The employee was standing on a 6-foot ladder to remove a three-way mixing valve from the ceiling pipes and change the valve stem. As the mixing valve was loosened, hot hydronic fluid (water/steam) poured out through the valve stem and splashed onto the employee's skin and clothing. The employee sustained second- and third-degree burns to their chest and stomach. They also sustained first- and second-degree burns on their left hand, arms, and thigh.

An employee was repairing a dump truck tailgate when a finger on his right hand was crushed between the metal base and the hook for the chain.

An employee was operating a drill press when their gloved hand got caught in the drill, resulting in an amputation of the top portion of his right index finger.

An employee was struck by a reversing excavator resulting in broken bones in the right leg.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
