On November 19, 2024, at approximately 2:40 PM, an employee was using a vertical bandsaw to trim excess material on a life ring. The tip of a finger on the employee's left hand contacted the blade, resulting in an amputation.

On June 2, 2024, at 12:40 PM, an employee was bending brackets using a brake press when his fingertip was caught between the bracket and the wall of the machine. The employee sustained a fingertip amputation. The machine was not guarded at the time.

An employee got on top of an aseptic food and beverage carton filling machine to remove a piece of loose metal that was in the way when the machine cycled and the shear contacted his hand, resulting in a left index fingertip amputation.

An employee was operating a stand-up forklift to close doors around the facility. As the vehicle was traveling in reverse, the forklift struck a wall and the employee's left foot was pinned between the forklift and the wall. All of the toes on the employee's left foot were amputated.

Two employees were moving an axle tube. Employee 1 set down his end of the tube; the injured employee dropped his end. The injured employee's index finger was caught between the falling tube and another tube, and he suffered an amputation to the finger.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was riding a pallet jack toward the breakroom when the pallet jack struck an order picker and the employee's foot was fractured.

On September 13, 2024, the injured employee was walking through an aisle where a co-worker was removing pallets from the racking system using an order picker. The corner of a pallet being moved contacted the corner of a skid next to it, causing two empty pallets to fall 12 to 16 feet from the racking onto the injured employee. The employee sustained a laceration and contusions to the back of their head and left shoulder.

An employee was operating a panel saw. The machine malfunctioned and the employee went behind it to try and fix it. When he adjusted a valve, the machine activated and moved forward trapping him between the beam and the wood preparing to load. The employee sustained fractured ribs and injury to the abdomen and chest areas.

An employee was cutting wood parts on a band saw. The saw cut her left index finger at the knuckle closest to the palm and the employee sustained an amputation to the left index finger.

An employee was clearing a jam in a T-nut insertion machine when it cycled, resulting in a left index fingertip amputation.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
