Final narrative
At about 12:20 a.m. on March 11, 2020, an employee was trying to adjust a stack of sheet metal when the stack fell. The employee's left ring finger was pinched between the sheet metal and a pallet, and the fingertip was amputated.
At about 9:45 a.m. on March 10, 2025, an employee was using a 5-ton overhead crane to move a right-hand check fixture, weighing 5,150 pounds, in a coordinate-measuring machine lab. As the crane lifted the fixture, the metal sling securing the fixture slipped over the employee's left thumb. The thumb was crushed between the metal slip and fixture, and the last joint was amputated.
On September 19, 2024, an employee was assisting with a die changeover on a 2,000-ton press when the press was cycled, causing their hand to be pinched and resulting in an amputation to their ring fingertip.
On May 13, 2024, an employee was polishing a piece of metal using a lathe when his glove became caught in the metal part pulling his hand into the machine. A few of the employee's fingers were fractured and dislocated.
On January 10, 2024, at approximately 6:00 p.m., an employee was attempting to remove a part that was stuck to the conveyor of an 800-ton press. The employee's right index finger was pinched between the part and the conveyor guide, resulting in a fingertip amputation.
On November 15, 2023, at 11:30 PM, an employee was walking through the facility. He went through a gate and went a further 2 or 3 steps when his right foot was run over by a forklift. The employee's foot was crushed and required hospitalization and surgery.
