Final narrative
An employee was working inside a gondola rail car, rigging bundles of steel rebar for hoisting out of the car. During a partial breakout lift (hoisting about 2 feet to gain access for slinging), a nylon strap parted under the 1,100-2,200-pound load, which fell back into the car and struck the employee. The employee suffered a broken femur and collarbone.

The slabs (approximately 4 feet by 20 feet) were being hoisted by a powered industrial truck (PIT) equipped with an electromagnetic lifting device. The lifting device failed and released a steel slab that struck the employee's right foot. The employee sustained injuries to their big toe, including partial amputation.

At approximately 11:00 a.m., employees were working at Manchester Terminal onboard a vessel. Approximately 12 employees were loading 5' x 10' steel plates onto the vessel (BBC Plata). The ship's crane was loading the plates inside the vessel with assistance from the employees. During the operation, two of the injured employee's fingers got pinched between two plates. He pulled away, and his left-hand, middle ring fingertip was partially severed (degloving).

The valve was cracked, and some spent acid still in the line sprayed out. The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

The employee sustained bruises and contusions to their right shoulder and chest.
