Kraton Chemical, LLC.

An employee was venting a 275-gallon intermediate bulk container to facilitate gravity feeding. He stepped up onto the platform of the secondary containment to gain access to the center cap. While doing so, he lost his footing and fell from the platform (25.5 inches above grade) to the concrete floor and suffered a fractured left tibia and fibula.

Kraton Chemical, LLC.

An employee was using a 15-foot metal rod to clear aluminum chloride from a hopper into a reactor below. After the aluminum chloride fell into the hopper, back-pressure caused the aluminum chloride to come up through the hopper and strike the employee in the upper chest and neck. The employee was hospitalized with third degree burns to his neck and upper chest.

An employee was trying to open a pastillator basket filter (turns resin product into quarter-inch beads) when the pressure built up underneath the filter, causing the liquid resin to splash onto and burn his face and right forearm. He was hospitalized.

An employee was walking in an aisle when a forklift backed up over her foot, resulting in a skin injury that required surgery to repair.

S. H. Bell Company

An employee was looking into the bucket of a front-end loader to see how much material was in it. A skid steer backed into the employee, who then fell into the edge of the front loader's bucket and suffered internal bleeding in the abdominal area.

SHM Newport Shipyard, LLC

An employee was spotting for a forklift operation. The forklift backed over the employee, who suffered a complete amputation to one lower leg and a partial amputation to the other leg. The employee was hospitalized.

An employee was cleaning his work area and preparing it to begin making air-set molds when a nearby forklift that was backing out from a load swung around and struck the employee. They sustained a lower left leg fracture.

An employee drank water from a water cooler in the control room that was contaminated with sodium nitrite. The employee sustained methemoglobinemia and was hospitalized.

An employee was collecting waste material and taking it to a waste container that was in close proximity to material that was emitting chemical fumes due to elevated temperatures. The employee inhaled the fumes and was admitted to the hospital due to the exposure.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
