An employee was removing a belt from a motor on an extruder when his finger was pinched by the belt, resulting in an amputation to the first knuckle.

A temporary employee cleaned a tripper and radioed the operator to restart the machine. The employee was then caught in a rotating shaft, suffering a left shoulder dislocation and left arm fracture that required hospitalization. The guard was removed at the time of the incident.
