An employee was standing on the wheel well of a sweeper vehicle. He was checking the radiator water level immediately after shutting the machine down. As he was filling the radiator with water, it began to bubble up, so he dismounted the machine. He landed oddly on the ground and fractured his left leg above the ankle.

On May 16, 2017, an employee was assisting a welder transition a pipe when he suffered from heat stress and required hospitalization.

An employee was standing next to a bore pipe observing the work. An excavator was being used to pull an auger bit out of the bore pipe. A chain was attached to the excavator to pull the auger sections. While the excavator was pulling the auger, the chain failed and whiplashed into the employee's face. The employee lost the left eye and suffered a right orbital socket fracture.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was acting as a spotter for a forklift operator. While its forks were being raised, the forklift came into contact with a power line. The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

An employee was leaning down to install conduit. When he stood up, a 2-inch all-thread rod caught and lacerated his left eyelid.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was standing on the ground awaiting the lowering of a pipe. After the pipe was lowered to the ground by a skid steer, it rolled and struck the employee's left leg. The employee was hospitalized with a fractured leg.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
