An employee was removing a piece of plastic from a conveyor line when her right hand was caught between the conveyor roller and a nip point, resulting in a right ring fingertip amputation.
An employee was squatting to inspect a dough mixer. She struck her head and lost her balance, causing her to grab onto a piece of machinery. The machine crushed her right index finger, resulting in an amputation.
An employee was assembling a vertical dough lift machine which required four bolts to assemble. The employee installed the top two bolts. While attempting to align the holes and install the bottom two bolts, the machine shifted and the employee's left little finger was caught in a pinch point resulting in amputation after the second knuckle.
