An employee was taking out old parts of a 690-volt electrical panel. When the employee removed one of the panel's doors and placed it next to the live side of the panel, an arc flash burned both of the employee's hands. The employee was hospitalized.
An employee was installing a ground wire to a power transmission pole. The ground wire contacted an energized portion of a cut-out, causing an arc-flash. The employee was hospitalized with second degree burns to their chest and arms.
An employee was connecting a utility transformer for underground service to a home. The employee's impact drill went across two connection bars with 240 volts of potential, creating an arc flash. The employee sustained burns to the face and eyes due to the arc flash and molten aluminum.

An employee was working to move a paper roller carriage, which is a 3 by 3-foot paper roll weighing approximately 500 pounds. As the employee moved the carriage across a floor track, the paper roll was dislodged and caught the employee's finger, severing it. The employee's right middle fingertip was partially amputated.
