On August 15th, 2025, an employee was aligning/connecting a coupler to the generator of a wind turbine. The coupler was secured in place by a strap that was connected to the turbine's chain hoist and a bolt. While lining up the remaining holes, the coupler rotated while the employee's right index finger was in one of the bolt holes, causing a partial amputation through the phalanx.

Greenbrier Rail Services

An employee was using his left hand to push an axle down the line on a raised track work area while holding the remaining axle back with his right hand. A finger on his right hand was caught between the rail and the dust guard of the axle, resulting in a fracture and a laceration.

New Millennium Building Systems

An employee was moving metal out of a cart. He had removed one piece and was working to move another when the metal shifted. His right ring finger was crushed between the two pieces, and he suffered a partial amputation to the finger pad at the tip.

Triad Metals International

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

Renegade Group LLC

Employees were replacing a 24" pig trap door. A strap was shackled on the pig trap door and connected to a forklift. Welders began to cut the door off. An employee was holding onto the door when the cut was finished. The door shifted and caught his fingers, resulting in a hand injury and amputation.

Universal Stainless & Alloy Products Inc.

An employee used a crane to set down a bar. The bar measured 9 inches by 16 feet. The strap got stuck under the bar, and the employee asked another employee to jog the roll. The bar rolled and pinched the injured employee's left hand, resulting in a fingertip amputation.

Two employees were hospitalized for electrical shock and burns after contacting energized electrical components of a turbine system.

Vestas Blades America, Inc

Four employees were traveling in a utility task vehicle (UTV) on the east lot of the site. While turning, the UTV flipped over onto the driver's side. The driver's left wrist and hand were crushed by the frame of the UTV resulting in a fractured and dislocated wrist, as well as hand injuries. The driver was hospitalized and required surgery.

TAKKION Operating LLC

Employees were preparing to unload a wind turbine gearbox from a flatbed trailer. An employee went to reposition himself on the flatbed trailer and grabbed onto a wooden piece of the shipping cribbing for support. The piece of wood began to come loose, causing the employee to lose his balance. The employee jumped to the ground about 4 feet below and landed on his feet. His left ankle rolled and the employee was hospitalized with a fractured ankle that required surgery.

Vestas Blades America, Inc

An employee was working to move a paper roller carriage, which is a 3 by 3-foot paper roll weighing approximately 500 pounds. The employee's hands were positioned in the area where the spool meets the cradle. As the employee moved the carriage across a floor track, the paper roll was dislodged and caught the employee's finger, severing it. The employee's right middle fingertip was partially amputated.

Solar Turbines Incorporated

An employee was carrying items while descending the stairs outside the building. The employee fell and sustained a T12 vertebra fracture.

Concrete Industries Nebraska City

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

Elkhorn Mechanical and Combustion

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

Cellar Crew LLC

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

BANB Farms Inc.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
