An employee was installing a square duct while standing on an 8-foot A-frame stepladder when he lost his balance and fell to the ground. He required stitches on his left eyebrow area and broke his left wrist and T12 vertebra.

An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
