An employee was running a tagline of an elevated load (conveyor bed) during an installation. As the load was lifted, the tagline became twisted around the employee's thumb. As the load moved, the rope caused an open wound to the right thumb that resulted in a thumb tip amputation.
An employee was on a step ladder, troubleshooting an issue with a conveyor, when he lost his balance and made contact with the conveyor belt and pulley. The employee's left little finger was amputated.
