An employee was running a tagline of an elevated load (conveyor bed) during an installation. As the load was lifted, the tagline became twisted around the employee's thumb. As the load moved, the rope caused an open wound to the right thumb that resulted in a thumb tip amputation.

An employee was on a step ladder, troubleshooting an issue with a conveyor, when he lost his balance and made contact with the conveyor belt and pulley. The employee's left little finger was amputated.

On June 17, 2019, an employee was painting on an elevated platform when the power went out. The employee fell 10 feet 6 inches to the floor and was injured. The employee was hospitalized for facial fractures.

On June 6, 2018, around 9:00 a.m. an employee parked and engaged the breaks of a loaded forklift on a slight incline and got off the forklift to verify space availability in the cargo van that was being loaded. The forklift rolled forward crushing his legs between the bumper of the cargo van and the material on the forklift causing fractures to both legs. The employee was hospitalized.

An employee was attempting to track a conveyor belt when his jacket was pulled into the conveyor belt, breaking bones in his right arm. He was hospitalized. The conveyor was not locked/tagged out at the time.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An installation technician for a wind turbine lift was using a chain hoist to move materials up a tower. The chain storage unit overflowed; the chain spilled and fell about 40-50 feet down the tower. It struck the injured employee in the face, causing fractures to their nose and cheekbones.

On November 25, 2024, at 9:00 a.m., two employees were moving a metal staging pallet that was located in an automation cell. The injured employee slipped and their left middle finger was crushed between the pallet and metal frame of the machine, resulting in an amputation to the finger padding.

On October 22, 2024, at approximately 10:15 a.m., an employee was forming a stand leg on a standard brake press. As they were readjusting the part, the tip of their right thumb was caught in the brake press, resulting in a partial amputation. The incident occurred during training.

An employee was cleaning welding debris from a steel part that was in a fixture. The part moved, cut through the employee's cut-resistant glove, and amputated their right middle fingertip.

An employee was changing the dies of a brake press. They loosened the securement bolts on the upper die (approximately 6-feet long and weighing 200-pounds) and the upper die fell out of the press. The employee attempted to catch the die and their left middle and ring fingers were caught between the upper die and the bottom die, resulting in amputations.

An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee tripped on a rotor on the floor of a store, fell, and suffered a broken right thumb.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.
