An employee was attempting to cut a small piece of tape that was taped around multiple wires with a splicing knife when he was shocked/electrocuted. His entire right arm tensed up all the way up to his shoulder, causing him to drop his splicing knife.

An employee was installing an electrical box when they became pinned between the bucket of an excavator and the electrical fiber box. The employee sustained a laceration to their left inner leg.

An overhead rigging chain with a splice box broke. An employee was struck in head by the chain sling resulting in a contusion and skull fracture.

A temporary employee and two coworkers were attempting to secure a tailgate onto a dump truck when one of them lost control and dropped the tailgate. It fell down on the employee's hand, amputating his left middle fingertip (to the cuticle) and fracturing his ring fingertip.

Lott Brothers Construction Company LTD

A crew was working near an electrical pole. A co-worker was using a front-loader to grade a slope when the loader slid into a guy-wire and became tangled and stuck. The injured employee used a handheld grinder to cut the wire a few feet from the ground. The lower part of the wire fell to the ground, but the upper part of the wire fell toward the pole and contacted an energized part before the end landed in nearby bushes. The employee attempted to shake the bush to free the wire so it could fall back toward the pole. The wire popped up and contacted his hands, resulting in electrical entry burns to both hands and exit burns on both feet.

Quality Service Team, LLC

On November 18, 2023, two employees were using a 2x4 to lift a power line. The boom lift they were operating made contact with power lines and both employees sustained electrical shock injuries.

Jemez Mountains Electric Co-Op, Inc

An employee was part of a crew that was servicing a power line. The employee was in a bucket truck when he contacted the 7,200-volt power line. The electricity entered through one hand and exited the other, causing several electrical burns. The employee was not wearing proper personal protective equipment at the time.

Commonwealth Edison Company

An employee was helping a crew set up a 45-foot pole between two other poles. It was being set up between two energized lines and the pole made contact with the A-phase. The employee was electrocuted and lost consciousness. The electricity also caused an entry wound in their left forearm and an exit wound in their left foot.

CJE Rebar Corp

An employee was handling a 45-foot long rebar for installation on the third floor of a building at a 30-foot elevation. The rebar came into close proximity of a powerline situated 13 feet off the building. The employee sustained electrical burns to his hands from electric discharge, requiring hospitalization.

Fluor

An employee was working to free a load by grabbing and shaking the live side of a tugger line. Once the load became free and progressed upward, the employee's hand was caught and pulled into a snatch block. The employee's right thumb was broken and lacerated. The employee was hospitalized and required a surgical amputation.

UES Professinoal Solutions, LLC

An employee was operating a drill rig when when their hand got caught between a 1/4-inch steel cable and a drill rod, resulting in the amputation of four fingers.

Elecnor Hawkeye, LLC

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

WEST AFFUM DEVELOPMENT CO LLC

An employee was visiting a patient's home to fit them with a wearable cardiac defibrillator. As he was walking from the front door to his car, a large dog broke free from a chain and attacked him. He was bitten on the forearm and fell. The patient was then able to restrain the dog. The employee was hospitalized with severe dog bites and required surgery.

An employee tripped over a junior I-beam about 4 feet long and 6 inches tall that was installed for circulating water valves and fell to the ground, resulting in a broken right-side rib.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

Hobby Lobby Stores, Inc.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

Sugar Cane Growers Cooperative of Florida

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

Air-flo/Erwood Heating & Air Conditioning

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
