An employee was operating a chop saw to cut wood studs when the chop saw activated via the foot pedal, amputating the employee's left mid-forearm.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

The tugging motion caused the employee to lose their footing and step onto the sheetrock ceiling. The employee fell through the ceiling to the ground about 8 feet below and suffered a broken lower left leg. The employee was hospitalized.

An employee was going between a ladder and a wall to repair a wall in the utility room. A brad-nailing gun was hanging on a piece of wood on the side of the ladder. The employee contacted the brad nailer and the trigger was pushed into the wooden piece. The brad-nailing gun actuated and shot a 2-inch brad into the employee's abdomen.

An employee was on a scaffold caulking and painting a manufactured home. She fell off the scaffold an estimated 6 to 8 feet, landing on the concrete floor. The employee suffered upper body injuries and was hospitalized.
