An employee was operating a gypsum pump when he lost his footing and grabbed an area to prevent him from falling. The area he grabbed contained mixing blades that were rotating. As a result, he lost the end of his left middle and ring fingers just above the fingernail.

An employee was installing materials used for waterproofing on the roof of an apartment complex. The area was protected by a passive fall protection system with both the middle and top temporary railings installed. The employee was on his knees installing aluminum flashing and bumped into the middle railing. The middle railing failed and the employee fell 25 feet to the ground below, fracturing their right leg.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
