An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. An employee was on an extension ladder descending from a roof when the ladder collapsed and lowered. The employee fell approximately 20 feet to the ground, resulting in fractures to their ribs and left wrist.

The injured employee was acting as a spotter for another employee that was using a forklift to move crane plates. The injured employee went to walk on top of staged bracing poles that had been stacked. The top layer of bracing shifted when stepped on, causing the employee to lose balance and fall. During the fall, one of the bracing poles dislodged and landed on the employee s left leg, causing a compound fracture.

An employee was working to remove an existing window from a customer's home. The employee was descending an extension ladder when they fell backward onto a concrete patio. The employee sustained head trauma and fractures to their shoulder, ribs, and a vertebra.

An employee was painting the exterior of a house when they fell approximately 12 feet from a ladder. They landed on the ground and sustained a fractured right ankle.

An employee climbed a ladder onto a roof, then stepped from new roof sheathing onto a tarped section of old roof, and fell to the ground. The employee suffered fractures to their pelvis, vertebrae, and the left wrist. The employee was hospitalized.

The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was assembling a baker type scaffold. He was beginning to transverse down the scaffold, approximately six feet off the ground, when the scaffold tipped over and he fell. The employee was hospitalized with a fractured femur.
