Final narrative
An employee was standing on top of a ceiling joist in attic space to install an HVAC vent system. The ceiling joist cracked, split, and broke in half causing the employee to fall approximately 10 feet. The employee was hospitalized for a concussion and head, back and neck trauma.
Two employees were in a scissor lift approximately 17 to 18 feet high to hang duct work. They had finished hanging a piece when the scissor lift activated, began moving quickly in circles, and overturned. One of the employees was hospitalized with vertebrae, wrist, and ankle fractures. Both employees were tied off in the scissor lift at the time of the incident.
