An employee was returning a client to bed from the shower. The employee slipped and the client rolled out of the bed, landing on the employee's leg and fracturing the patella and fibula. The employee was hospitalized.

An employee was carrying dirty laundry to a laundry room when the lid to the clothes basket came off and fell to the tiled floor. The employee slipped on the lid, fell forward, and suffered a broken lower tibia and contusions to the head and knees.
An employee was attempting to lift a patient off the ground. The patient tried to help and pushed himself back onto the employee. The employee fell over the curb and the patient landed on top of him, resulting in knee injuries that required hospitalization.
An employee was moving a patient off one bed and onto another using a draw sheet to assist the patient with a "log roll." The patient rolled off the bed and onto the employee, pinning the employee's leg underneath. They sustained a right lower leg fracture.

Employees were working to block a client from eloping. The client and employees were running down the hall when the client pushed the injured employee, causing her to fall and slide into a steel doorframe. The employee sustained an injury to her left arm/shoulder.
