An employee was cleaning mud off the rear of his truck so that he could shut the tailgate when he slipped on mud and fell to the pavement. The employee sustained fractures to his left femur and the left side of his pelvis.

The injured employee was standing on the driver's side of a flatbed truck while another employee was using a forklift to load metal plates. As the forklift operator was removing the forks from the top plate, it began to slide. The injured employee's left middle finger was caught between the lifting eye of the two plates, causing his hand to be crushed between the top and bottom plates. He sustained a partial amputation to his left middle finger.

An employee was watching a front-end loader scrape frozen aggregate from a sidewalk. After the front-end loader had made a pass, the employee moved towards the sidewalk to begin melting frozen aggregate with a torch. The front-end loader began to make another pass of the sidewalk, causing the loader bucket to swing and strike the employee. The employee was hospitalized with a concussion and a strained neck.

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

An employee was using a utility knife to cut honeycomb paper when the knife amputated their left index fingertip.

An employee was cleaning a resident's chimney. While using a drill, the employee's glove became caught in the drill and their finger was fractured.

An employee was grinding some hardfacing on a bulldozer blade when the 9-inch grinder kicked back and lacerated his left leg above the knee. The employee was hospitalized and required surgery.

An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.
