Final narrative
An employee fell from the second floor of a creel rack, suffering a broken left femur.
An employee was operating the crosscut machine, which uses table extensions for cutting larger rugs. To unfold a table, the employee pushed in the left bracket and then went to push in the right bracket. When he pushed in the bracket that is underneath the table, the extension came down, catching his fingertip between the middle and right table extensions. This resulted in a partial amputation and a fracture to the fingertip.
An employee was removing shelving from a storage rack. The employee was positioned on the shelving when the rack became unstable as screws were removed, which caused the employee to fall approximately 6 feet to the floor below. The employee suffered a fractured left wrist.
