SUPERIOR ESSEX

At 5:40 p.m. on August 28, 2025, an employee was traversing cable onto a reel when the line sped up. The cable caught his glove and pulled his left ring finger onto the spool, causing a partial amputation to the finger.

An employee was cleaning a hopper when an extruding screw caught the tip of his right index finger. The fingertip was amputated.

An employee was repositioning the print tape that prints labeling on a cable. The tape stuck to his finger when he tried to position it on the cable, causing his finger to get caught between the print wheel and the cable resulting in an amputation injury.

An employee was attempting to load a reel in a machine. While he was lining up the reel with the pintle and operating the controls with his left hand, the reel shifted and smashed his right-hand fingers between the reel and the pintle. His right index fingertip was amputated as a result.

An employee's thumb was partially amputated while performing maintenance on a primary machine. Four of the employee's fingers were caught in a belt and pulley.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was attempting to remove a tarp from a flatbed trailer when the trailer handle struck the employee in the chest, resulting in two broken ribs and a punctured lung.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

On June 4, 2025, an employee was starting up the rotary winding machine and breaking off a piece of conduit when his left arm was struck by a moving part. The employee was hospitalized with a severe laceration to the elbow that exposed the tendon and the joint.

An employee was using a handheld grinder on a metal part. Sparks flew off the part and caught their clothing on fire. The employee sustained severe burns to their face, neck, and underarms.

An employee was operating a brake press and amputated a fingertip.

An employee was operating a roll press machine when the material began to jam. The employee was pushing the material through when a part of the roll came loose and their left hand went through it and caught a piece of the roll. The roll of steel cut their left index finger resulting in a partial amputation.

An employee was operating a standup forklift in the warehouse to move it out of the way when the forklift struck the adjacent shelving. The employee sustained a broken hand that required surgery and hospitalization.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
