An employee was hot air welding PVC roof seams and transitioning to a new location on a roof when his extension cord bumped a gallon container of acetone. The container began to tip and he tried to catch it with his right hand. Some contents spilled and were subsequently ignited by a flameless hot air welder that was approximately 2 feet away. His pants and shirt caught fire, and he received second degree burns to his right hand, arm, and bicep. He was hospitalized.

An employee side stepped and then tripped and fell. When he reached his arms out to brace for impact, his right arm entered a mop cart of hot asphalt and sustained burns from the top of the hand to just below the elbow.

An employee was cleaning a cordless drill. As they applied brake fluid to the drill and pulled the trigger, the fluid ignited. The employee suffered burns to the face, hands, and arms.

An employee was welding on a rack when sparks ignited residual diesel on his pants. The employee sustained first-, second-, and third-degree burns.

An employee was spraying glue to the floor of an enclosed cargo trailer. The fumes made contact with the heater causing a flash fire. The employee suffered burns to their face and right hand.

An employee was replacing a valve in the gas line to the fryer. The shutoff valve malfunctioned resulting in a gas leak that then ignited. The employee sustained burns requiring hospitalization.

An employee was using a torch when a hole in the oxygen line leaked gas that then ignited. The employee sustained burns to the left palm and wrist.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

UPSON COMPANY

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee fell through a hole in the ceiling and suffered injuries to his side and right arm. When he fell, a piece of concrete fell with him and struck his arm. He was hospitalized.

An employee was laying glue on the corner of a three-story commercial flat roof to apply new thermoplastic polyolefin roofing material. He lost his balance and fell at least 35 feet to the concrete ground below. The employee sustained fractures.

An employee was installing roof tiles. The employee fell about 6 feet and suffered a back injury.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
