On September 30, 2020, an employee had just replaced a motor and belts for an aspirator fan blower motor. After tensioning the belt, the employee was rotating the assembly sheave to confirm the belt was properly seated. The employee's right index finger was caught between the belt and the sheave and the fingertip was amputated.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
