A machine operator was cleaning the conveyor belt when the cleaning pad was caught between the belt and pulley. The employee's gloved hand was then pulled into the moving belt and pulley resulting in amputation of an index finger.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Bauer Foundation Corp.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

The injured employee removed a front plate to clean a pump. His left hand was caught in moving parts and the employee's middle fingertip was amputated.

BERKSHIRE STERILE MANUFACTURING, INC.

An employee was working on a work assist vehicle lift that was elevated about 3 feet off the ground. The employee fell to the concrete floor and hit their head. The employee suffered a broken disc in the back, as well as a broken skull with internal bleeding.

Trulieve, Inc.

An employee was removing a cross-threaded nut and bolt from a metal machine using a torque wrench when the wrench slipped from the nut. They struck their right index finger against the machine, resulting in crush injuries to the finger and a fingertip amputation.

An employee was working on a bottle molding press. When she was adding a part to the machine, it amputated part of her right middle finger.

An employee was cleaning the mezzanine area when they crossed over the railing onto an I-beam and fell through a ceiling tile. The employee fell 15 feet to the floor and sustained a fractured spine and left heel.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
