An employee was operating a forklift in reverse. As she was turning from the main aisle to the side aisle, her right thumb was crushed between the vehicle and a rack and was lacerated.

An employee was reading the numbers on the racks to find a skid that needed to be transported. As the employee was walking along the rack, they came in contact with a skid of glass. The employee's left forearm was lacerated.

On January 19, 2023, an employee was making adjustments to the support bar of the glass sanding machine when their left foot became pinched between the top and bottom conveyor belts of the machine. The employee was hospitalized.

An employee was moving a section of glass from a motorized pallet jack into a vertical washing and inspection machine. The jack moved unexpectedly and the section of glass fell off the pallet and pinned the employee against the washing and inspection machine. The employee was hospitalized with fractured ribs.

At 7:22 a.m. on July 31, 2022, three employees were in a glass gantry, cutting pieces of glass to deposit them in a dumpster. One of them was putting a piece of glass in the dumpster when it bounced back and cut the employee's neck below the ear.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was attempting to remove a tarp from a flatbed trailer when the trailer handle struck the employee in the chest, resulting in two broken ribs and a punctured lung.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

On August 16, 2025, at approximately 11:15 AM, an employee entered the "pack cell" at a glass fabrication line to unjam glass product from the "pop up". The employee was removing glass product when residual pneumatic energy completed cycling the "pop up". The employee's fingers became caught in the pinch point created between the "pop up" and the conveyor. The employee sustained lacerations and tendon damage to their left index, middle, and ring fingers.

On July 5, 2025, an employee was working to lubricate the chain on an autoclave machine in the coating factory. Their left index finger was caught in the machine resulting in surgical amputation at the first joint. The machine was guarded at the time, but was not locked out/tagged out.

An employee was walking through a facility after returning from break when a forklift ran over their left foot. The employee was hospitalized with fractures to their foot and ankle that required surgery.

An employee was greasing a furnace when their left index finger was caught in a gear and amputated.

On March 4, 2025, an employee was walking when they tripped over a rolled up work mat and fell to the floor. The employee sustained a fractured hip.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
