A rolling mill was shut down due to a cobble between two mill stands. The injured employee was cleaning the cobble from the delivery side of stand A, while another employee went to the control panel to move stand B and stand A moved instead. The injured employee's right thumb got caught between the delivery guide of stand A and the looper trough. The tip of his thumb required surgical amputation to the bone to allow for stitches.

A truck driver was walking on top of a load of logs to remove tarps. He approached the edge to prepare for dismounting. He stepped on a sideboard that shifted, causing him to fall to the ground. He sustained a cut to the top of his head that required five stitches, fractures to his right forearm, and a dislocated right elbow.

An employee entered the baghouse to take dust samples from a compartment. When he entered the building there was baghouse dust approximately 12 inches deep on the floor. The employee walked through the dust about 5 feet when he noticed the layer of dust beneath the top was hot and felt his feet burning. Heated dust had entered the top of his boot and worked downward until resting underneath the arch of his right foot causing burns to the bottom of his foot.

An employee was using a crane to move a 6-by-60-foot bundle of rebar from a stock rack to production lines. The employee placed the crane remote on an adjacent bundle and was placing the wire sling under the suspended bundle when the remote control slid off the adjacent bundle and fell under the suspended bundle. The employee reached under the suspended bundle when the bundle tie wire suddenly broke. The bundle fell onto the employee's right hand, resulting in a crushed and fractured hand.

An employee was working between railcars on an active railroad track to connect the airbrake. Another employee moved the railcars, striking and running over the employee resulting in amputation of both legs.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was working to assess the operation of a high-pressure water nozzle. The water system was activated and the employee was cut by high pressure water on his left hand where the palm meets the wrist. The employee was hospitalized.

An employee was performing torching work when metal fell on top of their foot and fractured the smallest toe.

An employee was working to push items in a trailer dumpster with his feet; his back was against a building wall. He fell to the concrete ground, resulting in fractures.

An employee was guiding a high-pressure water hose through a 4-inch gap on a horizontal condensing tube when the hose blew apart. The hose left the opening and the fitting at the end of it struck the employee on the side of his face. The employee sustained blunt force trauma to the left side of his head, including a laceration from his left temple through his ear.

On July 24, 2025, an employee was shoveling material when a piece of steel slag fell from a nearby furnace and struck them in the back. The employee was hospitalized with fractures to their thoracic vertebrae and a metatarsal.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
