An employee was showing an apprentice how to lock down the metal to a cut-off saw machine when the machine was engaged as he was placing his right index finger on a slot. His finger was caught between the slot and the machine's chain, resulting in the amputation of his right index fingertip without bone loss.

An employee was holding a sheet of metal with a coworker while another coworker was operating a brake press machine. One of his coworkers told him to move the brake press's pedal away from their work station. As he moved the pedal with his foot, he lost balance and caught himself by resting his right hand on the brake press. At the same time, the press engaged and amputated his right pinky finger.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee was driving a forklift when its right front wheel dropped off a 6-inch curb. The forklift rolled over and trapped him against the concrete surface. He suffered a broken pelvis, ribs, and collarbone, as well as a collapsed lung.

An employee was operating a brake press when the top die broke, causing the bottom die to roll out and onto the employee's legs. Both of the employee's legs were crushed below the knee, and both legs were partially amputated. The employee's left leg was also fractured above the knee.
