An employee was on top of a block wall, cleaning off debris. He fell off the wall to the ground (concrete and hard-packed earth) below, breaking both feet and his right fibula.
An employee was cleaning a rotating drum when his hand was caught in a pinch point between the drum and the hopper resulting in a partial amputation of the right ring finger.
An employee was washing down his truck when his right middle finger was caught in a pinch point between the drum and the hopper of the truck resulting in a fingertip amputation.
