An employee was walking on top of a piece of cargo when the employee stepped through a section of plastic sheeting covering the cargo. The employee fell approximately 14 feet to the steel deck of the ship's hold and fractured their left shoulder, left ankle, and left wrist.

The employee spotted a strap on one of the loads and went to remove it by placing his left arm between the forklift's attachment and the load. The forklift driver started to move and the employee's left arm got pinned between the forklift and the load, resulting in compartment syndrome.

The employee sustained fractures to their back and left leg.

The employee sustained bruises and contusions to their right shoulder and chest.
