They heated the tip of the rebar with the torch and put it inside the tank. An explosion and a fire occurred. Two employees were injured; one employee was hospitalized with fractured bones and first- and second-degree burns.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant.
