Final narrative
An employee was walking down a set of steps on the grading line. A leg from a pallet rack was blocking their path and when the employee tried to step over the obstacle, he missed the next step and fell forward onto the floor. The employee sustained a broken left leg.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.
An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.
An employee was using a water hose to clean debris out of the outfeed of a log conditioning vat. Water began entering the vat from the adjoining vat through a void in the separation wall at the infeed of the vats. As the employee was exiting the vat he had been working in using the access opening at the outfeed, hot water exiting the access opening entered the top of his protective hip wader. It pooled at the bottom of the wader and burned his left foot and ankle.
