The employee was struck by the falling pallet of 30-pound cardboard boxes filled with poultry products and he sustained fractures to his right tibia and fibula that required surgery.

Two employees were performing preventative maintenance on a machine. The injured employee noticed excess grease on the drive system and wiped it off with a rag while the machine was turned on and running at a slow speed. The rag was caught and the employee's hand was pulled into the drive system resulting in a partial amputation of his right index finger.
