An employee was walking through the store room when her left foot caught on a dolly. She lost balance and fell forward onto the floor with both arms outstretched in front of her. The employee sustained a facial fracture by the left eye socket and a fractured left arm.

An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
