An employee was performing a visual inspection of the pulley cables in the penthouse above sterilization. The employee used a paper towel to assist in checking for burrs and defects in the cable by folding and positioning the paper towel around the three cables of the pulley system. The paper towel caught on the cable, pulling the employee's right hand into the pulley and causing a partial amputation of the little finger (distal phalange) and an avulsion injury to the ring finger.

An employee was clearing a jam and jogging the equipment due to a "barrel trip gate fault." The operator noticed a large jam and entered a gloved finger into the point of operation to clear the jam while jogging the machine. The left index finger was caught in the dial (part of the machine that brings the plunger up into the barrel for assembly of the syringe) and the fingertip was amputated. There are interlock doors on the machine normally guarding this point of operation, but the interlocks did not prevent the machine from functioning in the jog mode with the doors open. The jog function is controlled by a two-button control that is operated from the handheld pendent hardwired to the machine. Both job buttons can be operated with the same hand.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.

An employee opened a flammables storage cabinet to get something out, smelled a chemical odor, and began to feel unwell and dizzy. She was hospitalized, having been exposed to formic acid from a leaking bottle at the bottom of the cabinet.

The injured employee was cleaning a pre-mix blend from a high-speed mixer. Another employee working in the same room opened the pneumatic valve for the machine. The injured employee's right forearm was crushed in the mixer when the discharge valve closed. He was hospitalized.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.
